Disability Case Management

We combine clinical insight with real-world work solutions as our experienced Vocational Rehabilitation Professionals work collaboratively with all stakeholders (including clients, employers, healthcare providers, and insurers) to identify barriers early, create individualized recovery plans, and facilitate timely reintegration into the workforce. Whether your client is on short-term disability, long-term disability, or is a complex case, we tailor our services to meet their needs and your objectives.

Our case management process includes:

  1. A comprehensive file review and initial interview that includes (but is not limited to) their medical information, functional abilities and limitations, occupational information, identification of return to work barriers and opportunities, medical disability guidelines and a predictive analysis of expected recovery times.

  2. The development of a comprehensive case management plan in alignment with the terms and conditions of the client’s disability plan and benefit approval period.

  3. Following approval from the referral source, the vocational rehab consultant will begin goal-setting with the client to set expectations regarding engagement, recovery, and return-to-work preparation.

  4. Throughout the duration of the plan, the vocational rehab consultant will facilitate approved services, maintain regular contact with the client and their treatment team, and provide regular updates to the referral source, while concurrently monitoring the forecasted resolution date.

Ongoing case management includes, but is not limited to:

  1. Regular engagement with the client.

  2. Regular plan monitoring to ensure that the plan aligns with the individual’s current needs, goals, and abilities.

  3. Facilitating plan adjustments by considering alternate or additional strategies that positively impact the individual’s function.

  4. Communicating and coordinating the individual’s plan with healthcare providers, including regular updates on recovery progress or potential regressions that may introduce or impose new limitations, which need to be addressed within the case management plan.

  5. Engaging with the referral source to provide regular progress updates, as well as timely updates regarding any unexpected deviations or irregularities to the plan (including compliance issues, medical recovery aberrations, non-medical factors, etc.).

  6. Preparing ongoing progress updates, funding requests, and formal recommendations for the referral source.

For complex or stalled claims, strategic insight can make the difference between prolonged disability and a clear path forward. Our Complex File Review and Case Management Plan Development service offers insurers a comprehensive, cost-effective tool to evaluate recovery potential and next steps.

Led by a senior IVRS consultant, this service includes a detailed review of the complete claim file, clinical documentation, and vocational history. We conduct a critical analysis of medical, functional, and psychosocial factors and develop a customized Vocational Rehabilitation Report and Case Management Plan tailored to the claimant’s unique needs.

Each report provides clear, actionable recommendations, along with projected costs, enabling the referring agency to either implement the plan independently or partner with IVRS for full case management support. This service is especially effective in determining the need for further assessment (e.g., medical, cognitive, or functional), the potential benefit of treatment interventions, and the optimal timing for return-to-work planning.

Change of Definition (COD) Screening

Change of Definition (COD) transitions are pivotal points in long-term disability claims, and early planning is essential to reduce the length of the claim and optimize RTW outcomes. IVRS offers a proactive COD Screening service, ideally conducted 9 to 12 months before the COD date, to equip insurers with the data and direction needed to make informed decisions prior to COD.

This screening includes a comprehensive review of the individual’s vocational profile (education, work history, training, English proficiency, and transferable skills) alongside objective benchmarking of their computer competencies. When combined with confirmed medical restrictions and an appropriate target wage, this data forms the foundation for a focused, pre-COD plan aimed at identifying viable alternate occupations and preparing the individual accordingly.

Our process helps minimize surprises at the COD milestone by laying the groundwork for either a sustainable transition to alternate employment or a clear rationale for ongoing claim eligibility.
