The RISE (Resilience, Integrated Support, and Education) Program is designed to empower individuals struggling with, or recovering from, mental health challenges (including substance use disorders) to successfully re-enter the workforce.

This program supports individuals in building personal resilience to overcome mental health challenges and thrive in the workplace by offering a coordinated approach to mental healthcare, utilizing the Collaborative Care Model, Measurement-Based Care, and open communication with Primary Care Providers.

Each mental health claim is different. Shouldn’t the support be too?

Mental health claims aren’t one-size-fits-all, yet many return-to-work approaches treat them that way. RISE was designed to be different.

We recognize that traditional case management often falls short for individuals struggling with mental health challenges. The RISE program delivers wraparound services that integrate a deep understanding of mental health with vocational rehabilitation, creating a truly tailored recovery pathway for each client.

The RISE Program is designed to:

  • Improve return-to-work outcomes by delivering coordinated, personalized care that addresses each client’s unique mental health needs.

  • Reduce claim durations through proactive interventions and consistent communication between all stakeholders.

  • Lower return-to-work costs by identifying mental health barriers early and resolving them before they escalate.

  • Promote client independence through the RISE Education Program, which equips individuals with the tools and knowledge to understand, manage, and self-regulate their symptoms.

How it Works

The RISE Program uses the following models and methods:

Collaborative Care Model

Through this integrated, team-based approach, we ensure that mental health needs are addressed by maintaining open communication among the client and all stakeholders, which leads to faster, more effective outcomes for those navigating mental health challenges.

Mental Health Continuum

We recognize that mental health is not static -it exists on a spectrum and can fluctuate over time. Using the Mental Health Continuum model, we can identify changes in symptoms, evaluate their impact on function, and tailor support strategies that promote recovery.

Measurement-Based Care

We measure changes in symptom severity and frequency over time, using real-time data to guide treatment decisions. By regularly assessing client progress, we make timely adjustments to care plans, ensuring that interventions align with the client’s needs.

Education Program

RISE Education is an 8-week group-based program designed to empower participants with the knowledge, tools, and community support they need to better understand and manage their mental health. Through guided sessions, individuals gain insights on a variety of mental health topics, build personalized care strategies, and strengthen their readiness for a sustainable return to work.

Program topics include:
- Understanding, self-monitoring, and regulating mental health symptoms
- Planning a successful and sustainable return to work following a mental health leave
- Nutrition, exercise, and sleep for mental health
- Managing responsibilities and stress through self-care practices
- And more!

While it is recommended that clients involved in the RISE Case Management Program also be referred to the RISE Education Program, anyone can participate in the Education Program, regardless of their current Case Management provider/program.
