Rise. Recover. Return.
The RISE (Resilience, Integrated Support, and Education) Program is designed to empower individuals struggling with, or recovering from, mental health challenges (including substance use disorders) to successfully re-enter the workforce.
This program supports individuals in building personal resilience to overcome mental health challenges and thrive in the workplace by offering a coordinated approach to mental healthcare, utilizing the Collaborative Care Model, Measurement-Based Care, and open communication with Primary Care Providers.
RISE Mental Health Case Management Program
Mental health claims require more than a standard return-to-work approach. The RISE Program is designed to support individuals with mental health conditions through integrated, personalized care that blends vocational rehabilitation with a deep understanding of psychological recovery.
What sets RISE apart:
Customized recovery pathways: Tailored support plans that reflect the client’s mental health condition, personal goals, and psychological abilities.
Proactive case management: Early identification and resolution of mental health barriers to shorten claim duration and improve recovery outcomes.
Collaborative communication: Consistent coordination between all stakeholders to ensure everyone is aligned throughout the recovery and return-to-work process.
Client empowerment: Encouragement of independence and self-management through participation in the RISE Education Program.
RISE is not just about returning to work; it’s about restoring wellness, confidence, and the capacity to move forward.
RISE Education Program
The RISE Education Program is a group-based mental health education course designed to equip participants with the tools, strategies, and support needed to understand and manage their mental health.
Whether returning to work or building readiness for future employment, participants gain valuable skills to promote resilience, improve self-regulation, and plan for a successful recovery.
Key topics include:
Understanding and managing mental health symptoms
Developing a sustainable return-to-work plan
Nutrition, physical activity, and sleep for mental wellness
Managing stress and responsibilities through self-care
And more!
RISE Education is available to all individuals, regardless of whether they are currently enrolled in a RISE Case Management Program. Referring participants to both services can offer added benefits and continuity of care.

How it Works
Collaborative Care Model
Through this integrated, team-based approach, we ensure that mental health needs are addressed by maintaining open communication among the client and all stakeholders, which leads to faster, more effective outcomes for those navigating mental health challenges.
Mental Health Continuum
We recognize that mental health is not static -it exists on a spectrum and can fluctuate over time. Using the Mental Health Continuum model, we can identify changes in symptoms, evaluate their impact on function, and tailor support strategies that promote recovery.
Measurement-Based Care
We measure changes in symptom severity and frequency over time, using real-time data to guide treatment decisions. By regularly assessing client progress, we make timely adjustments to care plans, ensuring that interventions align with the client’s needs.
